NEW PRESCRIPTION CHANGE IMPORTANT INFORMATION EFFECCTIVE JANUARY 1, 2023
Starting January 1, 2023, CDPHP/Capital Rx will be our new pharmacy benefit manager (PBM) replacing ProAct. PBMs manage prescription drug benefits on behalf of health insurers. You’ll continue to have access to national pharmacy chains and supermarkets, including CVS, Walgreens, Walmart, Rite Aid, Price Chopper/Market 32, Hannaford, Kinney Drugs, and Wegmans, as well as most other pharmacies you’ve had in the past.
Because of this change, you will receive a new CDPHP member ID card in the mail which includes new pharmacy claim processing information. Be sure to use the new ID card at your pharmacy and doctor’s office starting January 1, 2023.
After January 1, 2023, you can view your prescription benefit coverage details by logging in to your member account at member.cdphp.com.

Benefit eligible employees will use the forms and guidelines below for their health, dental and vision insurance. PLEASE NOTE: If you are adding dependents to ANY of your insurance coverage, you MUST supply Birth and Marriage Certificates for each dependent.
